Minor Illness and/or irritation
The Animal Clinic of Windermere is happy to be able to offer our clients the ability to board their furry family
members within a medical facility. Due to this fact, we put forth great effort to notice even small changes while
your pet is with us. Often our staff will find minor irritations or illness such as ear and skin infections. We prefer
to start treatment on right away, but these minor issues COULD wait for your return. If one of these minor
conditions is noticed by our staff, please let us know how you would like for us to proceed:
There are times pets become ill while boarding and require immediate medical care. In the event that my pet
becomes ill and requires medical attention during his/her stay, the staff at Animal Clinic of Windermere will make
every effort to contact me prior to treatment. If contact cannot be made, I understand that necessary steps will
be taken to treat my pet, and I agree to be financially responsible for the cost of medical treatment.
*In the event that my pet requires medical or surgical treatments, I understand that the change in the level of care
and monitoring will require hospitalization fees rather than boarding fees.
I agree to and fully understand the above statements.
